It means reclaiming the authority to design, fund, and deliver health care in ways that reflect what our communities need, grounded in our languages, our lands, our cultures, and our ways of knowing.

This work is about more than changing policy. It is about advancing First Nations self-determination in health. With direction from Chiefs and guided by the voices of communities, we are building a health system rooted in:

Health Transformation is how we move from surviving to thriving.

It is how we strengthen health and well-being, not only for individuals and families, but for our Nations as a whole.

Together, First Nations will define what quality care should look like and develop the tools, programs, and supports needed to put that vision into practice. This work will build on the strengths, knowledge, and priorities of our communities.
